| | NOTICE TO THE PUBLIC THIS NOTICE EXPLAINS HOW TO REGISTER A REPORT OF EXTRAORDINARY ACCOMPLISHMENT OR ACT OF SERVICE BY THE DEPARTMENT OR ANY OF ITS EMPLOYEES. Employees of the Police Department greatly appreciate written and/or verbal acknowledgements when their performance positively reflects upon the professionalism of the agency.
In-person or via telephone (847-255-2416), ask to speak to the employee’s immediate supervisor. He or she will document your comments in written form and place a copy in the employees’ personnel file.
Written acknowledgements should be addressed to the employee's immediate supervisor or the Chief of Police. The document will be forwarded to the employee and a copy shall be placed in his or her personnel file. The address is as follows:
